• Site Plan - two copies
• Blocking Diagram - two copies
• Septic tank permit
• Driveway permit
• Preinspection form
• Document showing size & age of mobile home, copy of title or purchase agreement
• Skirting Diagram Class A only - two copies
• Notarized Letter/Affidavit if required see below*
• Flood Zone Acknowledgement 
• Stair Details - two copies
• Floor Plan - two copies
  

*A notarized letter/affidavit is required:
    1. Mobile home is in a different name from the property owner 
    2. Property owner is pulling the permit under the installer's name 
    3. Mobile home installer is doing the set-up but the property owner is           
       doing the electric, plumbing, or mechanical work.
