The following permit forms may be printed, filled out and returned to Sumter County Public Works.  Mail the form or bring to our office for processing.

Driveway Permit*
Please note that the Driveway Permit Form uses both letter and legal size paper so it may be beneficial to print the pages separately.

*Prior to submitting Driveway Permits, please have driveway staked.
